Cerebral metabolite spectrometry as a predictor of disseminated sclerosis therapy

Purpose – to analyze cerebral MRI protone spectrometry results in patients with disseminated sclerosis in different periods of disease to determine matabolism peculiarities of cerebral tissue and substantiate therapeutic recommendations.

Methods and subjects. By means of cerebral MRI spectrometry 46 patients were examined. 12 patients had a diseases onset, 13 patients experienced remitted course, 21 patients experienced remission course.

Results. All patients experienced reduction of N-acetilspartat peak (in average10–23 %, p < 0.01), creatinine (15–30 %, p < 0.01), choline (15–30 %, p < 0.01). Inositole concentration increased for 73 % (p < 0.01). There was lipids peak in 82 % of patients. In case of diseases onset there was significant Inositole concentration and reduction of choline and creatine concentration. In case of remission there was insitole increasing and reduction of N-acetilspartat, occurrence of lipids peak. Changes of choline and creatine concentration was typical for old dimielization focuses. Reduction of N-acetilspartat was marked in old focuses.

Conclusions. Different metabolic disorders were determined in mieline mebranes and neuron axons, part of them can be reversible. Depending on period and type of diseases the spectrographic cerebral image was different. There is a necessity of phospholipin, acetile aspartat, creatine splitting correction, reduction of lactat acydose as a therapy.
