MRI spectroscopy (in vivo 1H MRS) for differentiation of Parkinson’s disease and parkinsonian syndromes

Purpose: study the percularities of the brain metabolism in patients with Parkinson's disease using MRS.
Materials and methods. Two groups of patients were studied using in vivo 1H MRS (MR appliance Magnetom Vision) with 1,5 T magnetic field tension. First patient group had 118 patients 43—73 years old (without dementia signs) that have Parkinson's disease 4—7 years duration. Second group (control) had 75 patients 18 to 73 years old without neurological deficit. Both groups had their in vivo 1H MRS spectrum of following brain areas: white matter (parieto-temporal area), grey matter (occipital cortex area), pale globe and black substance areas.
Results. Based on in vivo 1H data the content and T2 relaxation times of main cerebral metabolites were obtained from patients with Parkinson's disease (PD), parkinsonian syndromes (PS), and control group patients. That allows us to estimate age and regional peculiarities of brain metabolism in non-demented patients with PD. For patients with PD the significant decrease of N-acetylaspartic acid (NAA), creatine (Cr) and the increase of choline (Cho) peak areas in basal ganglia are observed. There is the significant reduction in NAA/Cr ratios in left compared to right temporoparietal cortex for the patients with PD compared with volunteers. Comparison MRS data in right and left temporoparietal cortex for patients with PD shows a significant reduction of NAA/Cr in left hemisphere and non-significant reduction of NAA/Cr in right hemisphere. There is no dependence of NAA/Cr ratios on motor disability or disease duration. There are no significant differences in NAA/Cho and Cho/Cr ratios for patients with PD and for healthy elderly in temporoparietal cortex, and significant — in basal ganglia regions.
Conclusions. 1H MR spectroscopy gives a new insight into brain biochemistry in patients with Parkinson's disease and is very useful when differentiating PD and multisystem atrophy with PS.
