Immunologic aspects of etiology and pathogenesis of epilepsy: unclaimed therapeutic opportunities

Despite the long-term researches, conducted in the world in the sphere of epileptology, the etiology of idiopathic epilepsy still remains to be unknown, while its certain important pathogenesis aspects remain undetermined.
However, due to the recent advances in clinical immunology and virology, a number of new scientific factors have been established, having allowed another vision on the nature of refractory epilepsy. It is referred to the establishment of a pathogenetic role of inflammation in shaping the epileptic focus, the detection of high titers of auto-antibodies to neurons' glutamate receptors in patients with epilepsy and verification of etiologic role of chronic herpetic infection in a number of cases with refractory convulsive syndrome as well as substantiation of neuro-immune synapse model, that explains epileptogenesis by disturbances in the mechanisms of a subtle interaction between the immune, glial and nerve cells in the brain tissue. It is all those aspects that the present survey is dedicated to.
